What you need to know

TRUE HEALTH protects the confidentiality of every patient's medical records. As a patient, you have the right to access your medical records.

The following information is for you, as the patient, if you would like to request a copy of any portion of your medical records. If you have questions about a bill, please visit the billing section of our website.

You may access your available records on the patient portal, which allows you to conveniently manage your personal health information. With an account, you are able to access the following health information:

• Electronic medical record
• Laboratory test results
• Radiology and diagnostic testing results

If the information required is not found in the patient portal or an official copy is needed, please follow the instructions below and complete the Request for Access to Health Information form to help us process your request. Please note, a written request needs to be provided prior to processing. We are providing this downloadable form for your convenience and we have also included additional information, contacts and frequently asked questions below.

Medical records requests must be made to the facility where care took place (a hospital or physician’s office). Please use the dropdown list below to find location-specific contact information.

To request copies of diagnostic images (X-rays, CTs, MRIs, sonograms, etc.) and for laboratory inquiries, please contact the location where you received those services; there may be multiple locations to contact for your records. Please see the FAQ below for more information.

There are a variety of ways to request your medical records, including:

• In person
  • Requests for treatment purposes (continuity of care) will be expedited, all other requests will be processed within designated timeframe.
• Telephone (completed documentation is required before release of any information).
